Applications

For Rent Assistance:

Rental Housing Support Program (Henry, Stark, Bureau counties)

For Family Development Apartments:

Fairview Apartments and/or Lakeland Terrace (Kewanee)
Countryside Apartments (Geneseo)

For Elderly and/or Disabled Apartments:

Parkside Apartments (Kewanee)
Hollis House and/or Washington Apartments (Kewanee)
Lincoln House (Galva)
Maple City Apartments (Geneseo)

To apply for housing:

  1. 1. Select and print the desired application from the list above.
  2. 2. Fill out the application completely and sign where appropriate.
  3. 3. The following documentation is required of each family member:
    1. A. Birth Certificate
    2. B. Social Security Card
    3. C. A photo ID or Driver’s License is required of each family member over 18
      Please do not send originals in the mail. Send a copy or stop by the office and we will make a copy.
  4. 4. Submit the application and documentation in person or by mail to:

    125 N. Chestnut St.
    Kewanee, IL 61443

If you have trouble filling out the application, please call us at 309-852-2801 and we will be happy to assist you.

What happens next?

After we receive your completed application and documentation, we will perform a background check. You will be notified by letter of the outcome of the background check and the next step in the process.
